How do I scan the QR code from the same phone?

You can't scan a QR code with the same phone's camera while it's on that phone's screen. Two options: scan the QR from another screen (a computer or second phone); or use 'Enter details manually' in the Add eSIM step and paste the SM-DP+ address and activation code.
